Couldn't be faker. I'm a dentist, and while the Le Bret piercing is pretty crazy (ie, you WILL lose your lower front teeth if you leave it in, dumbasses) this is way photoshopped.
first, the "window" is size shape and reflective capacity of a standard dental mirror.
second, if you know anything about jaw anatomy, it IS way too low. yes, you'd push the upper lip up, but this is just nowhere near in relation to where the teeth are compared to facial structure.
lastly, if you know anything about TOOTH anatomy, it's clear that you're looking at the lingual (ie, tongue side) of the lower front incisors there. leading me to believe someone just took a pic of a mirror IN the mouth (showing the back of those teeth) and photoshopped it in .

No, really, it makes sense. If you stretch that piercing, it would push up your lower lip. This is probably quite real.

The piercing has no depth, the only hing you can see is flesh. It is about as real as a three dollar bill.

To those of you who think that's not real, guess again.
That's a lens and it's 100% real. The teeth appear to be in a different position than they should and the shape is also distorted.
But so are your teeth when viewed through a drinking glass.

Don't believe someone just because they're a dentist, or a doctor. It doesn't automatically make you an expert.
Visit the BME site I'm sure you'd find most of it hard to accept.
